Electrophysiology of the retraction syndromes.

چکیده
With the aid of modern electromyography, numerous authors (Pabst and Esslen, I960; Sato, ig60; Orlowski and W6jtowicz, I962; Burger, I963; Blodi, van Allen, and Yarbrough, I964; Huber and Esslen, I969) have recently come to the (surprisingly) unanimous conclusion that a paradoxical innervation of the external rectus muscle of the affected eye represents the pathogenetic principle of all the retraction syndromes (Stilling-Turk-Duane) (Duane, I905) and that the explanations given by the majority of earlier authors based on mechanical concepts (birth injury, congenital or acquired musculo-facial anomaly, etc.) are either insufficient or are at variance with the facts (Kriiger, I969). In this connection it must be emphasized that to accept a disorder of the antagonistic interaction of the extraocular muscles as the primary cause of the retraction syndromes does not exclude anatomical alterations of the muscular tissues, the more so as primary innervational disturbances are known to induce pathological changes in the muscles. An analysis of published cases (Lyle and Bridgman, I959; Malbran, 1953) and of our own series, has enabled us to distinguish three types of Duane's retraction syndrome:

متن کاملA Case Report of Congenital Myasthenia Gravis Presenting With Respiratory Distress
Congenital Myasthenic Syndromes (CMS) are rare inherited disorders characterized by dysfunction of neuromuscular transmission at the neuromuscular junction. Most patients with congenital myasthenic syndromes present in the infancy. Major symptoms of affected individuals include weakness and fatigue during the first years of life. Patients may show hypotonia, facial weakness, swallowing difficul...
متن کاملTwiddling in cardiac resynchronization therapy: 'when length matters'.
We report on a 75-year-old woman who successfully underwent cardiac resynchronization therapy. Two weeks after implantation, right continuous pectoral stimulation appeared. Retraction and dislodgement of the right atrial and right ventricular leads as well as coiling of the three leads around the device generator were observed on chest radiography, but, surprisingly, the left ventricular lead r...
متن کاملEvaluation of Patient's Dose and Estimate of Cancer Risk in Electrophysiology Studies and Ablation in Cath Lab Center of Afshar Hospital, Yazd, Iran
Introduction: Today electrophysiology studies and ablation have been developed due to increasing arrhythmias disorder of heart. In these diagnostic – treatments methods, the use of fluoroscopy can be causes patient radiation dose, therefore evaluation of patient's absorbed dose is necessary to protection of the radiation. The aim of this study was to evaluate the absorbed dose in patients under...
متن کاملCryoballoon dysfunction indicated by abrupt temperature drop during atrial fibrillation ablation.
Pulmonary vein (PV) isolation remains the cornerstone of atrial fibrillation (AF) ablation. Cryoballoon ablation is shown to be non-inferior to radiofrequency ablation in terms of efficacy and safety among paroxysmal AF patients. In this case, we report an intraprocedural adverse event encountered during cryoballoon ablation. A 46-year-old male patient with symptomatic, paroxysmal AF underwent ...
